Holland Bloorview Kids Rehabilitation Hospital aims to provide a welcoming, inclusive and equitable environment for all of our clients and families. Health equity means that everyone receives the same quality of care regardless of their personal characteristics or background, such as race or ethnicity, gender, where they live or how much money they make.
Understanding who we serve is important to improving our care and removing any barriers that exist. That is why we are asking our clients and families to participate in a new survey and tell us what they think about equity, diversity and inclusion at Holland Bloorview.
The survey is voluntary and anonymous (participants are not asked to provide their names). The survey includes 21 questions and takes about 25 minutes to complete. The survey is open to Holland Bloorview clients over the age of 14 years and all families until June 30, 2016
The information we gather from the survey will be shared with our Family Advisory Committee made up of parents of clients, as well as our client and family integrated care team, strategic plan committees and senior leadership at the hospital so that we can identify ways to improve our care environment and ensure it is as inclusive and equitable as possible.
Hospitals in the central Toronto area are already collecting information about personal characteristics and background in a survey. Holland Bloorview’s new survey asks some similar questions in addition to questions specifically about our hospital.

Hardcopy versions of the survey are available at the outpatient reception desk on the second floor at the hospital.
