Occupational Health and Wellness
Vaccination consent
This year, all employees, whether or not you choose to get vaccinated or are vaccinated off-site, are required to complete the flu declaration form. The declaration form is also available at the on-site flu clinics or through Occupational Health & Wellness in room 5E130.
Getting your flu shot off-site
You can also get the flu shot at your family physician's office; participating community pharmacies and Toronto Public Health flu clinics (visit www.toronto.ca and search "flu clinics" or call the city's information line at 311).
If you choose to get the flu shot in the community, please submit proof of vaccination within one week of being vaccinated. Staff can drop off proof to Occupational Health & Wellness in room 5E120 or fax it to 416-424-3847. Staff can also email a photo of the documentation, however, you are still required to drop off the declaration form to Occupational Health & Wellness.

Meditech Staff Gateway is your central hub for your employment information. This self service online portal gives you access to your pay stubs, T4s, benefit information, and provides the ability to submit updates to your employee file.
