Program: Adapted Recreation & Sport
Referral criteria:
- Holland Bloorview client
- Ages 5 - 18 years (up to 21 if in high school)
What the programs actually do: Inclusive recreation, including: wheelchair basketball, all abilities soccer, sledge hockey. Please visit website link above for further details.

Program: Adapted Recreation Equipment Loan Program
Referral criteria:
- Holland Bloorview client
- Ages 5 - 18 years (up to 21 if in high school)
What the programs actually do: Rent new adapted equipment for free. Examples of equipment include: a hockey sledge, adapted bike, swings and hammocks, hand propelled bicycles, portable bubble tube, and more. Please visit website link above for further details.

Program: Friendship & Belonging
Referral criteria:

- Ages 7 - 18 years (up to 21 if in high school)
What the programs actually do: 2 virtual groups with games, music, friendship & conversations. 1 month commitment.

Program: Employment Participation
Referral criteria:
- Holland Bloorview client
- Ages 14 - 19 years (up to 1 full year of high school to go)
What the programs actually do: volunteering or employment experiences, job coaching, personal consultations.

Waitlists period: 3 months
Program: Post Secondary Transition
Referral criteria:
- Holland Bloorview client
- In grade 11, or beginning of grade 12
What the programs actually do: explore school options, accommodation, and funding planning for college or university.

Waitlists period: 1 year
Program: Youth Engagement
Referral criteria:
- Current or former Holland Bloorview client
- Age 14 - 29 years
What the programs actually do: Improve hospital program and clinic experiences. The Youth Advisory Council (YAC) provides a sounding board for staff, students, clients and families to develop and evaluate Holland Bloorview programs and services. Youth Mentors and Youth Facilitators can directly support hospital programs and support.

Program: Bridging to Adulthood
Referral criteria:
- Current or former Holland Bloorview client
- Ages 14 - 18 years (up to 21 if in high school)
What the programs actually do: Learn about and meet adult programs and services. Personal planning consultations and support with applications. Family workshops to plan for life when high school ends.
