Who we are


The Post Secondary Transition Service provides support to students with disabilities who are currently in grade 11 or higher and are planning to attend college or university in the future. This service is provided by an Occupational Therapist. 

Who we serve


We can provide support to students with disabilities who are currently in grade 11 or higher and are planning to attend college or university. Clients may be eligible for service through their first year of transition after high school. 

What we do


The Occupational Therapist offers consultative support with gathering information and developing skills to help high school students prepare for a successful transition to post-secondary education. This can include collaboration with the student, family, school team and community partners.

Headed to college or university? Register for our post-secondary education & career virtual workshop series

Designed for youth:

  • In high-school (grades 10, 11, 12 or 12+) and working towards your OSSD
  • Living with a disability and/or an autism spectrum disorder
  • Living in Ontario

Parents/guardians/families are welcome to participate.

Webinars include a mix of information (presentation style) and opportunities to ask questions. We engage Youth Mentors to share their lived experiences.

This session covers the application process, how to access services and supports, and funding options. The session was recorded on October 16, 2021. 

Recording- Setting up accomodations at college or university

This session reviews accessing services and accommodations at college or university, maintaining physical and mental health, and funding options. Hear lived experiences from two youth mentors. This session was recorded on February 24, 2022.

Recording - Career exploration

This session introduces you to tips from career planners that can help you map out your future. Two mentors share their real life experiences. The session was recorded on November 11, 2021.

Recording- Parents as transition coaches

This session is for parents and caregivers of students who are preparing for post-secondary education. One parent and two post-secondary counselors share resources and tips for supporting young people at various stages of this exciting and challenging transition. This session was recorded on January 25, 2022.

Recording- Working or volunteering while attending college or university

Why? When? How? It's important to have practical work experience before you finish school. Learn how to get started. The session is built around a mentor's experiences with job search and working. The session was recorded on January 27, 2022.

Frequently asked questions


Are appointments virtual or in-person?
  • The initial intake meeting can happen onsite at Holland Bloorview or virtually. Follow up meetings are scheduled on an as needed basis and may take place at Holland Bloorview, the student's high school/college/university campus (within GTA), or virtually. 
What kind of topics are covered?

We can work on goals such as:

• Planning for academic accommodation needs at post-secondary 

• Assessing accessibility and equipment needs on a college/university campus and/or residence 

• Arranging for personal care support at college/university 

• Exploring funding options for post-secondary 

• Determining transportation options for college/university
