Foundation privacy policy 

Holland Bloorview Kids Rehabilitation Hospital Foundation values the trust you place in our mission and management and we recognize that we must be open and accountable in our treatment of the information we hold and receive. We are proud to demonstrate our commitment to the privacy of our stakeholders, by complying with the laws and regulations under, and principles of, applicable privacy laws in Canada. Our privacy policy outlines the principles and guidelines related to the collection, use, disclosure and retention of personal information. Electronic health record systems

Holland Bloorview uses the following provincial electronic record systems: ConnectingOntario Clinical Viewer and Electronic Child Health Network (eCHN). In addition, Holland Bloorview shares personal health information electronically with other health care providers for the purposes of delivering care to clients.

ConnectingOntario Clinical Viewer is a secure provincial web-based portal and eCHN is a similar secure pediatric electronic health record system. Both are used widely throughout the provincial healthcare system to provide Ontario clinicians and authorized members of a client’s healthcare team with real-time and streamlined access to digital health records (including lab results, hospital visits, dispensed dugs, diagnostic images and home and communication care information) and facilitate timely and coordinated client care.

As part of these services, Holland Bloorview discloses clients’ records to Ontario Health, which then become securely available to authorized users who are involved in a client’s care but may work at other healthcare organizations outside of Holland Bloorview via Ontario Health’s electronic health record (EHR) databases. Information is only used or disclosed for the purpose it was collected, unless otherwise consented for, or when it is required or permitted by law.

Photos and audiovisual recordings

To protect privacy, you must ask for consent before taking photos and audiovisual recordings of anyone on hospital premises and in appointments. Freedom of Information

Holland Bloorview Kids Rehabilitation Hospital is subject to the Freedom of Information and Protection of Privacy Act (FIPPA). We embrace FIPPA and are committed to working openly and providing information about our work. The Hospital must provide records on request, subject to limited exemptions as provided in the Act. Consent

At Holland Bloorview Kids Rehabilitation Hospital, the client, family and health-care team work together to make informed decisions about treatment choices. Treatment can include receiving medicines, check-ups, physiotherapy, life skills training and coaching. The goal is to help your child take part in making decisions about their health care, if they are able to make these decisions. Children and youth often rely on input from their family and health-care team to help in making difficult decisions. Learn about who gives consent.
