Transitions to Adulthood: Is your child ready?


Summary

We want parents/guardians of children ages 12 to 25 to complete a 10-minute survey one time telling us about their child’s readiness for the transition to adulthood.

Who can participate

Any parent or guardian whose child:

  • Is between the ages of 12 to 25
  • Has received pediatric rehabilitation services and/or transition services in Ontario

What's involved

This study is a one-time, online survey. We would like you to answer our survey about youth readiness for the transition to adulthood.

The survey will take between 10 and 20 minutes and all the information is in English. You will be entered into a draw to win one of six gift cards.
