What? Work with Transition Team members to develop your own Personalized Transition Plan to adulthood. Work through your own plan and receive coaching and support as you go!
Where? At Holland Bloorview - in the Family Resource Centre
When? Wednesday January 24th, 2024 6:00pm-7:30pm
Why? Transitioning to adulthood has many different moving parts. Come learn and make your own plan to make the transition as smooth as possible.

For Holland Bloorview Clients 17 years old and up (and/or their parents) who are looking to learn more and apply to Ontario Disability Support Program (ODSP). Learn the application process and the various supports available to you. If ready, receive 1:1 coaching to complete the financial eligibility application on the spot after the presentation.
