The Family Navigation Hub aims to share information and resources with families related to social needs such as stable housing, food and income security, access to childcare and more. 

The Family Navigation Hub is a designated student leadership experience and we work with Master of Social Work students completing practicums to support the delivery of Family Navigation Interventions.

Holland Bloorview is providing registered active clients an opportunity to participate in a social health check in to help us better understand their social needs. This is a voluntary process where families may be asked to participate in completing a questionnaire that helps identify any areas of social need.

Eligibility criteria: You must be a current Holland Bloorview client and be referred by a participating Holland Bloorview provider to be eligible for 
the 1:1 consultation service.
