Our care teams at Holland Bloorview make sure this happens at every appointment visit, in client rooms and waiting rooms, within screens for virtual visits, in the hallways and cafeteria lines, and we can’t thank them enough.
Jake’s Award
What is Jake’s Award for Amazing Health Care Providers?
Named for a client of Holland Bloorview, Jacob (Jake) Trossman, created by his family to celebrate exceptional clinical care. This clinician is someone who sees clients as a whole person with likes, dislikes, passions and interests. This individual delivers exceptional client and family-centred care, and is an outstanding role model and leader. The recipient of this award displays the values of compassion, equity, courage, resilience, excellence and innovation. In keeping with Jake’s spirit and sense of fun, Jake’s Award is a surprise celebration in the clinical setting. The award winner will receive a one-of-a-kind piece of art as a keepsake of this recognition.
Last year’s winner:
Brendan Price after being award Jake’s Award for Amazing Health Care Providers in May 2021. Brendan is against Acrylic, Glass Bead, Ceramic Stucco on Canvas, art by J.J. Smolkin.
Last year, Brenden Price became the first recipient of Jake’s Award. Brenden is Child Life Specialist, which allows him to teach youth practical strategies to cope with anxiety-provoking procedures. He was nominated by Lauren Kofsky on behalf of the Resource Team Bloorview School.
Upon winning, Brenden described, “I was completely surprised when I heard my name called as the recipient of the 2021 Jake’s Award. I was also honoured and proud to receive such an amazing award. I’d like to thank all the different team members for their support and guidance.”

Criteria:
- Understands Holland Bloorview’s vision of the most meaningful and healthy future for all children, youth and families.
- Ability to see each child or youth as a whole person with likes, dislikes, passions and interests.
- Outstanding role model and leader.
- Embodies a strengths-based approach to care.
- Establishes a special connection with clients and families.
Please include specific stories describing why this clinician shows the criteria listed above and note personal knowledge of the employee is not taken into account. Stories help scorers select the winner of the award.
Exclusion: Winner of any of the STAR awards or Jake’s Award in the last three years.

The DAISY Award
What is the DAISY Award?
The DAISY (Diseases Attacking the Immune System) award is an international recognition program that honours and celebrates the skillful, compassionate care nurses provide every day. Throughout the year, a Holland Bloorview nurse — nominated by their colleagues, students, volunteers and/or by clients and families—is selected to receive the DAISY award.
A DAISY Award recipient is a nurse who clearly understands our vision of the most meaningful and healthy future for all children, youth and families. Their clinical skills, and their ability to deliver exceptional child and family-centred care, exemplify the kind of nurses our clients, families, and staff recognize as an outstanding role model and leader. Furthermore, they embody the Holland Bloorview values of compassion, equity, courage and resilience, excellence and innovation.

Louise Rudden after being awarded a DAISY Award in 2021.
Upon becoming a recipient of a 2021 DAISY Award, Louise Rudden, nurse practitioner, explains, "It means so much to be recognized in this way and I will always cherish this award. It was an extremely emotional moment for me when I realized at the presentation ceremony they were talking about my work and accomplishments. It gave me a great sense of pride in my team and how they helped me achieve this great honour."
Maryanne Fellin after being awarded a DAISY Award in 2021.
Upon receiving the DAISY Award during last year's award ceremony, Maryanne Fellin, nurse practitioner, describes, "This recognition is one of the highlights of my professional career, for which I will always be grateful. I extend my appreciation and gratitude to my colleagues—without them, this honour would not have been possible."
Criteria:
Any registered nurse, registered practical nurse or nurse practitioner working at Holland Bloorview who:
- Exemplifies the vision, mission and values of Holland Bloorview Kids Rehabilitation Hospital
- Demonstrates extraordinary clinical skills and competencies in the delivery of evidence-based and holistic client care
- Demonstrates professionalism contributing to a work environment that promotes health and healing
- Establishes a collaborative partnership with clients and families, recognizing their unique experiences and needs
- Effectively engages clients and families in their rehabilitation journey, while being attuned to their unique narratives
- Advocates for patients and families in ways that enable them to make their voices heard
- Contributes to an environment that draws on and amplifies the strengths that clients and families bring in order to provide high-quality and safe care
- Is committed to advancing health equity and practices in a manner that celebrates the diversity of the community
- Is committed to lifelong learning and growth
