Respite registration process
Holland Bloorview has a two step process for accessing respite services.
Complete a respite application form
In order to register for any of our respite services you must first complete a respite application to confirm eligibility for services. Your application will be reviewed by the Respite team and you will be contacted as soon as possible to confirm eligibility.
You will only need to complete this application once per year. Return the form at any time during the year, either with your request for specific dates or in advance. Please return the application by mail, fax or in person to:

Register for specific respite dates
- Call 416-753-6066
- Choose Day or Overnight Respite
- Leave your name and telephone number as well as the dates you are interested in booking
- Provide a few options in case we cannot accommodate your first choice
Note: we welcome our in-patient clients to begin their respite visits on Mondays, Wednesdays or Fridays
Client registration services will contact you by phone and a confirmation letter will also be sent to you. Respite requests will be booked on a first come, first served basis.
Pay for the respite program(s) you have registered for
- Overnight respite - $55 per night
- Sunday respite day program - $55 per day
